Job: DSS / HAC Coordinator

This new role sits within our Age Well Early Intervention and Prevention Service, bringing together work from our Dementia Support and Hospital Aftercare teams. You’ll support older people across Salford — including those living with dementia or recently discharged from hospital — to regain confidence, independence and wellbeing. Using a strengths‑based approach, you’ll provide personalised, dementia‑informed support while working closely with colleagues and partners.

We’re looking for someone experienced in supporting older people who can work confidently, compassionately and as part of a wider team.

    You will bring:

  • Experience in health, social care or community support.
  • Understanding of dementia and the needs of older people.
  • Ability to carry out assessments and plan support.
  • Strong communication and relationship‑building skills.
  • Good organisational skills and confidence using IT systems.
  • Commitment to safeguarding, equality and inclusive practice.
  • Ability to work independently and with partners across health and community services.
  • Flexibility to travel across Salford (driving licence and vehicle required)
