10GM is currently mapping out the existing VCSE provision of hospital discharge and readmission avoidance support across Greater Manchester. This exercise aims to identify the needs of local communities with regards to hospital admission and discharge, and the gaps in current commissioning and provision.
10GM are asking VCSE organisations based in Greater Manchester who provide hospital discharge and readmission avoidance services and support to complete this form by 9.00am Friday 6 January 2022.
The mapping exercise will be concluded and published at the end of January 2023.
