You will work as part of our growing Children, Young People and Families Directorate and amongst a team of navigators based within local boroughs. The role will require you to work closely with local CAMHS services being co-located at the CAMHS service weekly. You will be responsible for supporting Children and young people with the transition from statutory services to community support that promotes their Recovery and sustained outcomes following completion of work with services such as CAMHS. You will carry a caseload of clients who you provide support to, at times and locations that support access and meet the needs of the client.

You will be familiar with the local community and emotional health and well-being services that are available for children and young people. You will be responsible for the development of relationships with partners from local statutory and community-based services. You will also be responsible for ensuring that the young people you are supporting receive a package of care and support that enables them to live in the community. The support you provide should be Recovery focussed, promoting independence and choice as well as encouraging autonomy. You will be responsible for ensuring the support you provide operates at capacity, fulfilling contractual obligations, and meeting the needs of CYP, families. You will ensure the needs of CYP are regarded with utmost importance through the delivery of services and ensure all contacts reinforce positive decision making, resilience and thriving mental health.

Contract: Fixed Term

Hours: 36 hours

Salary: £25.529.00
