GM care record

The Greater Manchester Care Record is a vital digital resource for the city region’s 2.8m citizens, that is used to help improve health and care services and save lives. It brings together your information from NHS and care services across all 10 Greater Manchester boroughs into one joined up record.

The GM Care Record allows workers in health or social care access to patient information, to help them understand a patient’s needs, and to support them in making the best decisions for your care. It shares important information about health and care including: any current health or care related issues, medications and treatments a patient may be receiving, allergies, particularly to medication, results of any recent tests or investigations, details on plans created for a patient’s care or treatment, and information on any social care or carer support.

Tameside Mental Health Forum
 

This forum is aimed at people with lived experience of mental health (including family/carers) to discuss key issues that surround mental health. It is a chance for individuals to regularly have their say on how services are being run, support areas of priority/focus and drive system and service developments and links to the Living Life Well Programme vision.

The next meeting is on Tuesday 23 November 2021 via Zoom.

Free NHS health checks

The Community Health Checks Programme is delivered by staff from Be Well Tameside which is part of Pennine Care NHS Foundation Trust.

Anyone aged 40 to 74, who lives or works in Tameside, or is registered with a Tameside GP is eligible for a full NHS Health Check provided they have not already had one at their GP practice in the last 5 years, and have not been diagnosed with cardiovascular disease, high blood pressure or diabetes.

Plasma for medicines campaign

You may have seen adverts about this campaign. Medicines made from plasma save and transform the lives of over 17,000 people in England each year.

Playlist for life

This is a project to raise awareness of the power of music to people living with dementia and their carers in local communities. The work is with the charity Playlist for Life that offers free resources and training to support people living with dementia, their families, and carers.

COVID-19

There are regular vaccination hubs around the borough, additional pop-up clinics and a vaccination bus. If you would like more information about the location and opening times for each, see www.tameside.gov.uk/covidvaccine. If you are eligible for a 3rd booster jab, you will be contacted by your GP or the NHS at the right time. There must be a gap of at least 26 weeks since your 2nd jab.
