Review of intermediate care provision in Tameside and Glossop

NHS Tameside and Glossop Clinical Commissioning Group (CCG) has launched a consultation, enabling residents of Tameside and Glossop to have their say on the way care is delivered for patients, usually older people, after leaving hospital or when they are at risk of being sent to hospital. This is known as Intermediate Care.
The aims of intermediate care are:
•Help people avoid going into hospital unnecessarily;
•Help people be as independent as possible after a stay in hospital; and
•Prevent people from having to move into a residential home until they really need to.
Intermediate Care in Tameside and Glossop includes two main elements;
‘Home First’ –  a range of services which support people in their own home or at a location in their local community and
Intermediate Care beds – beds for people coming out of hospital requiring a package of care which cannot be provided at home, or for people who need a short stay away from home for support to prevent them needing admission to hospital.
Currently, in Tameside and Glossop there are intermediate care beds located at the Stamford Unit, based within Tameside Hospital and Shire Hill Hospital in Glossop.
The CCG are consulting on 3 options:
Option 1
This option maintains the number of beds provided at the Stamford Unit (32) within the Tameside Hospital site and maintains the current community beds provided at Shire Hill in Glossop (36 beds). There is also access to 32 ‘discharge to assess’ beds at the Stamford Unit.
Option 2
All bed-based Intermediate Care would be provided at a single location in the Stamford Unit run by Tameside Hospital on their site in Ashton. The hospital is rated Good by the Care Quality Commission (CQC). The
provision of Intermediate Care beds at Shire Hill in Glossop would cease.
Option 3
This option would require us to work with private care home providers to develop capacity within existing care homes or invest locally in increasing capacity to host bed based Intermediate Care. This option would mean that Intermediate Care beds are not located in one single location but spread out across the area where capacity can be found. This option requires care home providers to be willing to invest in increasing bed spaces and if new care homes were required, a short-term solution would be required whilst capacity in the system is built.

To find out more about the options and to have your say please visit www.tamesideandglossopccg.org/intermediatecare
 You can pick up a paper copy of the consultation at GP practices across Tameside & Glossop or, alternatively, you can call the Healthwatch Office for a copy 0161 667 2526
Consultation ends 15 November 2017.

Picture of Health Focus this month - Tobacco

Public Health look at the way people in Tameside live, as a whole population.  Every month they are sharing facts and figures about health in Tameside, as well as giving us information on how we can help ourselves and the people around us to live well.
This month’s focus is on tobacco.
We hear lots about smoking related death, but what about smoking related life? Tobacco kills, but it causes illness that people have to live with first. There are at least 50 known conditions that are caused by tobacco. On average people in Tameside spend 20 years with a health problem before they die.
For those who die from smoking, they are likely to have lived with one or more of the following conditions first: Chronic Heart Disease and Stroke, Chronic Obstructive Pulmonary Disease, Vascular Dementia or Cancer.
Check out their infographic factsheet and use the information to help them, to help everyone improve their own health
